Warfarin has increasing indications in Family Practice as the population ages. Conditions such as atrial fibrillation without rhythm control are frequent in seniors. The same patients are on multiple medications, either prescribed or over the counter. This reference identifies these medications. There are also guidelines for dosage initiation and maintenance, along with procedures to manage over-anticoagulation.
Suggestions are made for frequency of monitoring and overlap of dosing following initial heparin anticoagulation.
